Stanford University Chronic Disease Self-Management Program (CDSMP) teaches consumers skills to manage their conditions and build their self-confidence so they can be successful in adopting healthy behaviors, improve communications with their physician and enhance their quality of life. It is designed to give older adults with chronic diseases, such as high blood pressure, diabetes and heart conditions; tips to help them make their daily life more enjoyable. Topics include: managing symptoms, fitness/exercise, nutrition, communication, medications, working with health care professionals and systems, and more. This program is recommended by AOA (Administration on Aging).
